Initial Comments:
A closure action was completed on June 30, 2023; for an effective date of July 1, 2023, as requested in the first facility correspondence dated June 30, 2023. In addition, there were a total of two revised facility correspondences subsequently received in the Central Office on July 3, 2023 and July 10, 2023. The facility request correspondence dated July 10, 2023 was accepted for this action. The purpose of this action was to discontinue participation under 42 CFR Part 441, Subpart D Regulations Emergency Preparedness in Psychiatric Residential Treatment Facilities. The total number of beds closed under this provider agreement is ten (10) certified beds.
